Founded in 1997, Santa Clara Family Health Plan (SCFHP) is a community-based health plan dedicated to offering affordable and high-quality health insurance to the diverse population of Santa Clara County. As a local, not-for-profit organization, it contracts with the State of California to provide health care services through various programs, including Medi-Cal, Cal-MediConnect (until its transition), and Covered California. Its mission is to ensure access to comprehensive health care for low-income families, seniors, and persons with disabilities, working in partnership with a network of local providers.
