Originally founded as Oö. Gesundheits- und Spitals-AG (gespag), the company was the central management and holding organization for public hospitals in the state of Upper Austria. In a major structural reform, Gespag was merged and rebranded into OÖ Gesundheitsholding GmbH (OÖG) to create a unified and integrated healthcare system for the region. As the largest employer in Upper Austria, its mission is to provide high-quality, accessible, and comprehensive medical care to the population through its network of clinics, medical centers, and educational facilities.
