Founded based on research from the University of Pennsylvania School of Nursing, RightCare Solutions, Inc. developed and marketed a software-as-a-service (SaaS) platform called D2S2 (Discharge Decision Support System). The company's mission was to reduce preventable hospital readmissions, improve patient outcomes, and lower healthcare costs by using evidence-based technology to guide medical professionals in creating personalized post-acute care plans. In April 2015, RightCare Solutions was acquired by naviHealth, a Cardinal Health company at the time, which has since become part of UnitedHealth Group's Optum.
