Originally founded in 1997 as a provider-sponsored, non-profit community-based health plan, Passport Health Plan was created to serve Medicaid beneficiaries in Kentucky. For over two decades, it focused on improving the health and quality of life of its members. After facing significant financial challenges, the plan was acquired by Molina Healthcare, Inc. and officially began operating as Passport Health Plan by Molina Healthcare on September 1, 2020. It continues its mission to provide quality healthcare services to the Medicaid population throughout the Commonwealth of Kentucky under the Molina brand.
