PACE Southeast Michigan is a non-profit organization dedicated to enabling frail seniors to live independently in their homes and communities. It operates under the national PACE model, which provides a full spectrum of healthcare services, including primary care, hospital care, home care, and prescription drugs, all through one integrated program. By providing and coordinating all needed preventive, primary, acute, and long-term care services, their mission is to help seniors maintain their health, well-being, and autonomy. The organization is a collaborative effort sponsored by Henry Ford Health System and Presbyterian Villages of Michigan.
